• Complete and sign the new account form (pages A, B, C & G are required to complete and sign, pages D, E, F & H are optional)
• Customer Agreement (please keep for your files-no signature required)
• Complete W9 / W8
• Sign risk disclosure(supplied separately)
• Complete REDI agreement
• Complete and sign exchange agreements including AMEX, NYSE, OPRA and NASDAQ
• Complete the ACAT form if you plan to transfer securities in addition to cash into your account.
• Include a copy of your driver’s license/passport
